Cohesion Phenomics offers several billing options for our clients. We work with our clients to facilitate easy and convenient billing.

Institutional Billing

Cohesion Phenomics can bill a referring clinic, hospital, laboratory, and physician directly if an Institutional Account with Cohesion Phenomics has been established. This allows the referring physician to submit samples through their facility’s send out laboratory. Establishing an institutional account is very simple. Insurance Billing

Cohesion Phenomics will file patient’s insurance. The patient is responsible for co-insurance, unmet deductible and/or non-covered services. Prior authorization is required for genetic testing. Cohesion Phenomics will obtain prior authorization, as dictated by the insurance carrier.

Self-Pay

Cohesion Phenomics provides discounted pricing and payment plans for self-pay patients. We accept checks, money orders, credit card payments, and wire transfers.

• Checks & Money Orders: Please make checks and money orders payable to Cohesion Phenomics. There is a $25.00 processing fee for insufficient check payments.
• Credit Card Payments: Approved credit card (Discover, MasterCard, or Visa) payments are accepted. The card type, cardholder name and signature, credit card account number, expiration date, and contact phone number must be provided.

Medicare

Cohesion Phenomics is not a contracted Medicare provider.

Medicaid

Cohesion Phenomics is a South Carolina Medicaid provider.
